To obtain a minor work permit, students/minors should gather the necessary information, a physician's certificate, and approvals from their potential employer and from their parents or legal guardian. Students may pick up work permit applications in reception area at Hamilton High School year round and the Miami School (140 Ross Avenue) from June 1 – July 31. After the appropriate forms are completed, by the student, parents, employer, and physician, the applicant (student) must personally bring back the completed forms to their school office; where it will be entered into the Ohio Department of Commerce - Minor Work Permit website. The student will then be issued their permit.
Office personnel will complete the permit register process this with the State of Ohio. Permits will not be issued or may be revoked based on school attendance.
